2367 LKH note:
In the newspaper article reporting Porter Chamber's death from spotted fever, it states that his brother died of the same disease back home.

That, plus the fact that Kelly does not appear in any records after the 1910 census, leads me to conclude that he and his brother, Porter, both died in 1908.

The first death at the hospital in some months occurred Monday, when Porter Chambers, of Athol, Breathitt County, died there from cerebro-spinal meningitis, or spotted fever. Brain troubles have been common in his family. His brother died of the same disease back home, and his father has had trouble. Besides, he got a bad blow on the head a few years ago, and that may have helped bring on the disease. His mother was down, and took the body home with her.

Looking for more documentation to confirm.

Find a Grave gives her maiden name as Pruitt
Of Clara and Richard Mullins' five daughters, 4 of their records in Kentucky Birth Index give Clara's maiden name as Pruitt, but one, Bernice Cordia Mullins, gives mother's maiden name as Spivey.

I have not yet found any marriage for Clara prior to her being wife of Richard Mullins in 1930, so that is not where the surname Pruitt comes from.

I have, however, found that Clara's mother Marvella Barnes married David Spivey in 1905. Clara was born in 1901. It appears that Clara became a part of Marvella and David Spivey's family and took the surname of Spivey throughout her life. I have not found any information about her biological father other than the surname of Pruitt.

Another possible clue.
In the 1900 census Marvella (age 22), seamstress, is living in Livingston in the household of David & Mary Wolf. Mary and Marvella were sisters.
Next door is the family of James & Mary Prewitt, whose household includes two sons, John (age 20) and Joseph (age 8), and two daughters, Clara (age 12) and Ellen (age 9).

This may be one of the reasons I have not yet found any record with exact birth information for Clara.

Spivey, Clara Angeline (I440)

Mary Frances Smith, first married James Ryles. They had at least three children.

This matches her application for widow's benefits for Isaiah Phipps Civil War pension for which she applied from Okalahoma on 16 January 1933. Isaiah first applied from Kentucky on 10 Oct 1882 for invalid pension benefits. Application No.462111. Mary's application No. 1729.982.
